Centurion offers a vision insurance plan through VSP.
The plan offers in- and out-of-network benefits, providing you the freedom to choose any vision provider. However, if you select an out-of-network provider, you may be responsible for paying in full at the time of service and submitting a claim for reimbursement. VSP does not issue ID cards; instead, you will use a participating provider and your Social Security number to determine eligibility. To find a participating provider, go to www.vsp.com.
The table below summarizes the key features of the vision plan. Please refer to the summary plan description for additional information on coverage and exclusions.
